CHC provides care for clients that are experiencing symptoms of dementia. Some clients may experience momentary confusion or lapses in memory, but others are dealing with advanced stages of dementia conditions. Specially trained CHC caregivers will provide activities that can improve a person’s quality of life such as music and other activities to stimulate the senses.
End of Life decisions are a delicate, personal family matter. It is a choice that is unique and individualized to the family and the client wishes. The decisions made for End of Life Care can provide memories and comfort for many years. Medicare covers hospice care, however, the care is for brief visits not full time care that some clients and their families require.
Chesapeake Home Care has trained staff that can provide Hospice Care for select periods of time to enable family to leave the client in a safe, caring environment, or 24 hour care if necessary. The schedule is planned according to the needs of the family members. The caregiver skills can range from a certified nursing assistant/home health aide to skilled care provided by a registered nurse. Your case manager will review your individual circumstances with you, (complimentary consultantion) and provide you with information to assist your family with life’s most important decision making.
Our compassionate CHC caregivers come to the aid of individuals who have been diagnosed with a terminal illness. We help them and their families with emotional and moral support as well as helping out around the home and providing personal and companionship care—whatever is needed.
CHC caregivers can support the family directly or work in partnership with a hospice agency or medical provider to fully meet the family’s needs.
Our ultimate goal is to help families make the most of their time together, helping to lighten the burden at a very difficult time. Through respite care, we give family caregivers needed rest.
And we can continue to help after a loved one’s passing. Our CHC caregivers help families deal with the grief and assist with tasks such as organizing personal items and moving belongings—or just being there to listen if family members need to talk.
Premature / Newborn / Pediatric Care
There are times in the life of your most precious family member that they will need care and you are unable to be there due to outside responsibilities and commitments. Or perhaps you have a child with special needs and you need someone to care for your child for a special occasion or a much deserved rest to rejuvinate yourself. Chesapeake Home Care has special trained home health aides or skilled nurses to care for your precious one. RN case manager will meet with family to assure that the care provider will be able to meet all the physical and emotional needs of the client. The caregiver will spend time with the child prior to the family entrusting full care so that the child is comfortable and familiar with the caregiver. All client care is supervised by a Neonatal-Pediatric Case Manager RN.
